By selecting the “I Accept”, you agree to the signing of the following Consent to Text Messaging (the “Consent”) electronically. You agree your electronic signature is the legal equivalent of your manual signature on the Consent. By selecting “I Accept” you agree to be legally bound by the Consent’s terms and conditions. You further agree that your use of a key pad, mouse or other device to select an item, button, icon or similar act/action, or to otherwise provide Futurity First Insurance Group LLC and its parent and affiliated entities (“FFIG”) your assent in accessing or making any transaction regarding any agreement, acknowledgement, consent terms, disclosures or conditions constitutes your signature (hereafter referred to as “E-Signature”), acceptance and agreement as if actually signed by you in writing. You also agree that no certification authority or other third party verification is necessary to validate your E-Signature and that the lack of such certification or third party verification will not in any way affect the enforceability of your E-Signature or any resulting contract between you and FFIG. You also represent that you are authorized to enter into this E-Signature Agreement.

Consent To Text Messaging

By selecting “I Accept” and entering my phone number, you authorize Futurity First Insurance Group LLC and its parent and affiliated entities (“FFIG”), specifically including but not limited to Senior Market Sales LLC, Insuractive LLC and Sequent Planning, LLC, to cause to be delivered to you text messages, including marketing of products and services, at the number provided with this authorization, including communications using an artificial or prerecorded voice, messages sent using an automatic telephone dialing system, or text messages containing audio or video texts. Text message and data rates may apply; text message frequency varies.

You acknowledge that you are not required to sign or agree to enter into this agreement as a condition of purchasing any property, goods, or services. In addition, you acknowledge and agree that you may opt-out of receiving communications for which you have herein provided consent by emailing privacy@seniormarketsales.com and specifically indicating that you no longer wish to receive text messages from FFIG for servicing/informational or telemarketing/advertising purposes, or both.
